Reproduction of this publication by any means, including E-MAIL, facsimile transmission, or any other electronic or print means, without the express written permission of The Bureau of National Affairs, Inc., is prohibited except as follows:

2) use you are planning, e.g., seminar handouts (include name, sponsorship, date, and location of seminar), republication (name of newsletter or other publication, general information on who will receive it), class materials, etc.
3) identification of who is requesting the permission, including name, title, organization name, address, telephone number, and fax number.
4) your deadline.
